Occupational Health & Safety Risk Assessment

Score a workplace hazard using the standard 5×5 Likelihood × Severity risk matrix, see where it falls, and get a matching control recommendation.

Assess a Hazard

List updates automatically based on the selected phase.
How probable is it that this hazard causes harm?
How serious would the resulting harm be?

SubstitutionReplace the hazard with a safer alternative.
3
Engineering ControlsIsolate people from the hazard (e.g. biosafety cabinets, ventilation).

Administrative ControlsChange how people work (SOPs, training, signage, scheduling).

How to use this tool

  • Select the Phase / Process Area first — the Hazard / Activity list updates automatically to show the specific risks relevant to that phase.
  • Rate Likelihood (how probable the hazard causes harm) and Severity (how serious the resulting harm would be), each on a 1–5 scale.
  • Risk Score = Likelihood × Severity. The matrix highlights exactly where your assessment falls.
  • Use the Hierarchy of Controls to select the most effective control(s) practicable for the risk level identified.
  • Don't see your hazard listed? Select a phase, then type it into the "Not listed?" field and tap + Add — it's added to that phase's list and selected automatically.
  • Once a full assessment is scored, tap "Save This Assessment" to keep it in the Saved Assessments list below, stored locally in your browser. Export the whole list as a CSV at any time.

Notes and limitations

  • This is a standard qualitative 5×5 risk matrix (consistent with ISO 45001 / HIRARC practice) — your organization's documented risk assessment procedure and rating definitions take precedence if they differ.
  • Risk assessments should be reviewed periodically and whenever a process, equipment, or hazard changes.
  • Document completed assessments (hazard, score, controls, reviewer, date) per your quality management system (e.g. ISO 15189 / ISO 45001).
